That means the $_POST variable is likely empty, if your form POSTing the data back?
It's not obvious from your form generator code.
When you view the page in the browser with the form, please view the source of that page and post it here.
Also the mysql driver is deprecated, it's really recommended to update to PDO for accessing the database.
