Code: Select all

<?php
if(isset($_POST['submit']))
{
  $size = sizeof($_POST['form_checkbox']);
  for($x=0; $x<$size); $x++)
  {
    echo $_POST['form_checkbox'][$x].'<br>';
  }
}
?>
<form method="post" action="<?php echo $_SERVER['PHP_SELF']; ?>">
<input type="checkbox" name="form_checkbox[]" value="one"><br>
<input type="checkbox" name="form_checkbox[]" value="two"><br>
<input type="checkbox" name="form_checkbox[]" value="three"><br>
<input type="checkbox" name="form_checkbox[]" value="four"><br>
<input type="submit" name="submit" value="submit">

</form>
