Hi, does anyone know of a PHP script that will build a web-based questionnaire that leads the user through a series of questions with A or B answers, please?

E.g.

Does the receiver have 1 or 2 inputs?

1 - Does replacing the cable fix the fault? Y/N
2 - Does swapping the connections change the symptoms Y/N

1Y - you have solved the problem.
1N - Does replacing the widget solve the problem? Y/N
2Y - Does replacing both cables fix the fault? Y/N
2N - Does replacing the receiver fix the fault Y/N

All you need to make a teapot stand is pegs and No Nails Glue.
