I am looking for a php survey script, which the user can create customised surveys from an admin interface, upload a database of potential respondents, generate an email with a UNIQUE url, or ID, so that individual responses can be captured for either further surveys or personalised marketing.
I would like to have the functionality to download a CSV or excel, with survey responses appended with the unique ID, so the original database matches with the responses (hope that makes sense). I've looked at lime survey, phpESP etc., but they dont seem to have this functionality.
