insert values into database using pdo

Hi I am a beginner php programmer and I have created a web form and it has validation as well. What I am trying to do is to put a pdo code into the script which will help me to insert the values into the database. The code you can find below..

Post the code in this thread and I'll take a loot. PDO is a great library to use because it supports Prepared Statements (for SQL Injection prevention).

Is it giving you an error? if so what error? if not, what is it returning that is different than you want or expect? "this is what i want, here's my code" is unfortunately not quite enough information
