I need a PHP contact form, with the following form entries:

Name, Answer, Email, Size, a tick box for Terms & Conditions, and Submit Button

Above the form there will be space to place an image.

520 wide, for implementation into facebook. (i can do this)

i can provide an image mock up if needed.
