Getting "error" with a basic PHP contact form

I'm getting a blank page with the word "error" up top. I added the form to my url. mydommain/contact.php and the PHP file in the root directory

The form:

When you send data from a form, you need to use $_POST['name'] to get the data that has been submitted, so in your case you need:

$mail_from=$_POST['customer_mail'];

// From
$header="from: $_POST['name'] <$mail_from>";


// Enter your email address

$send_contact=mail($to,$subject,$message,$header);


// Check, if message sent to your email
// display message "We've recived your information"
if($send_contact){
echo "We've recived your contact information";
}
else {
echo "ERROR";
}
?>
