i want to create an html form , that displays the product item quantity with a display select option

a php script that GETS the items and quantity selected

and puts the items in the ordered product table

i was also planning to GET the username and email to the oredered product table
any design help would be very helpful .. as i have never done this before... i dont even know if i am thinking it right ...
