PHP/mySQL site organisation?

I've succesfully mastered the basics of PHP and mySQL (which was not so heavy with my C++ background).

Now, I'm interested in a few basic organisation concepts:

How are PHP/mySQL based sites organized?

Is all data, such as text, reviews, stored in a database, or a plain html document?

I haven't found too many informations on the Net so I ask here, hopefully awaiting someone's answer :)

Every explanation, url or pdf :) is very welcomed

Is all data, such as text, reviews, stored in a database and only in db, but you may store some images in directory.....
At the question why? only cause it's faster, easy, and you not have a problem with access to files.
