Now i see the problem. The radio buttons must have the same name, like:

and than you'll have Yes or No inside your $_POST['dest'];

I never understood that kinda errors ...
I guess there's no aparent reason or specific situation for that to happen... it just happens and like you said: "it just crashes at random time" :)

It's been about 2 years since i don't use any Windows OS.

$LastNum is an array. Try print_r($LastNum);
The "1" output means that the mysql_query ran well.

I always suggest using unix timestamp for storing date/time information with the help of MySQL's FROM_UNIXTIMESTAMP function. Date/time field should be varchar ...
