I think you can not stop this problem with the code, It's a PHP related problem not a problem in your code.

Try to separate the hashing operation from the inserting and selecting operations. $password = md5($password); Then use the new $password variable in your sql statement. It's a PHP famous bug, Making reference on string offsets crashes PHP

If you're using PHP 5.3.0 so upgrade it to a newer version or rollback it to 5.2.0

You cannot convert PSD files into HTML ones according to my knowledge. The first solution is using Ajax/Jquery to change a specific row in the table.
and the second one is passing the information in the other rows to the page again to show them and it'll be a disaster for your server resources.
