You can run php scripts from the command line using php-cli.exe (found in your php directory). Just go:

zero based array

Are you sure you want the 30th element or the 30th index? Since you array will start at index 0, the 30th element is at index 29.

Failing that can I have a look at the data?

NTFS permissions

Replace token place holders

How about creating a word document that has place holders where you want actual data to go? You can them open this file in PHP and replace the place holders with values from the form.

fopen and fread

Hi,

You can probably fopen the url to the image file and use fread to get the entire file stored in a variable. Then fopen a new file on your server and fwrite the variable to that file. :D Phew! That should do it!

Check whitespace

A common problem I find when adding headers is if there is a whitespace before the starting PHP tag. For example: <?php header('Location: http://www.php.net/'); ?> This whitespace is counts as an output and is sent to the browser, thus generating the header error message.

Try using the $_SERVER array

The HTTP_REFERER value is stored in the $_SERVER (or $HTTP_SERVER_VARS in PHP < 4.0) so try using the following to get the value:

W32api

If you check out the W32api set of functions in the php documentation you'll see that it is possible to call a Windows API function from PHP. For example, GetUserName.

Use IFRAMEs

This can be done using an IFRAME tag on your page to contain the full size image. The main page can have all the thumbnails which are hyperlinks that target the IFRAME.

Temp directory or table in database

There are two possible options for this asyncronous operation. Either save the uploaded images to a directory and then have a page that enumerates that directory and gets a caption for each one from an HTML post.
